Sirte – Information System for the Territorial Network
Description
The aim of the information system for the Land Network (Sirte) is to ensure coordination of care pathways between different professionals and continuity of care between different levels of organisation, optimising integration between the hospital and the area.
The Sirte manages the processes within the care pathway, which take the form of a workflow involving access to the system through a single point which, after an initial identification of needs, starts the patient assessment phase, ensuring that the correct care path is identified (performance care, integrated home care, care in residential facilities), the definition of the Individual Assistential Plan (PAI) and the subsequent provision of care with periodic revaluations until the end of the care.
The system also provides for the possibility of introducing transversal routes, which allow an overview of all the services available and provided to the insured persons concerned (e.g. scheduled transport, Protesical Assistance, etc.).
Basically, the system consists of a multitude of computerised forms such as Single Access Point (SUP), Home and Performance Cure, Clinical Characters for Residential Facilities (Protected Residences, Assisted Health Residences, Hospice, Intermediate Cure, Community Hospital Hospital, Rehabilitation Facilities), the management of beds in health and social health facilities, ambulatory clinical carcass, supplementary assistance, major treasury, consultory, scheduled transport, protected divisions and routes shared with MMG/PLS via the web portal and electronic health records tools.
